Care Coordination

 

Care Coordination assists older adults (60 years and older) who have been hospitalized or suffered a health care crisis recently (within thirty days). The program provides short-term services to help our clients to remain independent and at home.

Our Care Coordinator completes an assessment in order to identify client needs and develop a care plan. Arrangement for appropriate services and follow up is managed by the Care Coordinator.

Care Coordination supportive services may include:


  • Homemaker services
  • Personal Assistance
  • Durable medical equipment not accessed by Medicare
  • Emergency Response System
  • Other services can address needs identified by assessment and care planning
