The Area Agency on Aging of the Capital Area and CAPCOG are rescheduling this program for a later date as a precautionary measure to assist with limiting the possible spread of COVID-19. Please check back for future dates of this program and others.
Living a Health Life with Chronic Conditions is a chronic disease self-management program that runs for six weeks. It teaches older adults, 60 and older, with chronic illnesses the skills needed in the day-to-day management of treatment and to maintain and increase one’s activities.
Using a group setting to give a shared learning experience, the program teaches techniques to deal with problems such as frustration, fatigue, pain and isolation. Program attendees learn about maintaining and improving strength, flexibility, and endurance. They also learn about the appropriate use of medications; how to communicate effectively with family, friends, and health professionals; how to manage their nutrition; and how to make decisions and evaluate new treatments.
Read the flyer to learn how to register.
