This training is for volunteers to become leaders in offering Chronic Disease Self-management programs in their local communities. The course spans a four day journey — June 3, 4, 17, and 18 — where they will learn to about the valuable lessons provided by Chronic Disease Self-Management programs, providing creative and engaging instruction, how to co-lead the program, and how evidence-based intervention courses can impact a person’s life. It also discusses the role’s leaders play in the CAPCOG region and the support CAPCOG provides for running programs.
Chronic Disease Self-Management programs were developed by the the Self-Managements Resource Center and are often taught in small group workshops for two and half hours once a week for six weeks by two leaders. The goal of the course is to teach people how to successfully navigate a chronic disease or illness to deal with symptoms, communicate with doctors, and live a better, healthier life. Topics covered in the program include:
- Appropriate exercise for maintaining and improving strength and endurance
- Falls prevention
- Healthy eating
- Better breathing techniques
- Appropriate use of medication
- Working more effectively with health care providers
- Communicating with friends and family
- Communicating with oneself
- Communicating with the health care system
- Action-planning, problem-solving, decision-making

Registration for this course is being handled by contacting Rhonda Thompson, AAACAP health and wellness coordinator. The course will take place over four days during multiple weeks of June.
To promote a professional environment, CAPCOG requires proper business casual attire. T-shirts, shorts, capri pants, and flip-flops are prohibited.
