Chronic Disease Self-Management Program is a six-week program that teaches older adults, 60 and older, with chronic illnesses the skills needed in the day-to-day management of treatment and to maintain and increase one’s activities. This program will be held from 10 a.m. to noon, Thursdays, from May 16 to June 20, 2024 at the Georgetown, The Caring Place.
Using a group setting to give a shared learning experience, the program teaches techniques to deal with problems such as frustration, fatigue, pain and isolation. Program attendees learn about maintaining and improving strength, flexibility, and endurance. They also learn about the appropriate use of medications; how to communicate effectively with family, friends, and health professionals; how to manage their nutrition; and how to make decisions and evaluate new treatments.
